Not all medical marijuana users in Oklahoma will be capable of providing for themselves. In cases where a user is homebound, older, critically ill, or a minor, having a designated caregiver becomes important because they will act as a legal agent in buying, transporting, and administering the drug for the person they represent. What Is an Oklahoma Medical Marijuana Caregiver?

According to the laws of Oklahoma, a medical marijuana caregiver refers to a licensed person whom an eligible patient nominates to help buy, hold, transfer, grow, and administer medical marijuana on behalf of the said patient. The Oklahoma Medical Marijuana Authority (OMMA) issues caregiver licenses independently of patient licenses.
It is essential to note that a caregiver license can be provided only if the doctor of the patient recommends, using the Official Physician Recommendation Form, that the patient requires a caregiver due to their health conditions. Just being a minor does not mean that one qualifies for a caregiver.   • Possesses up to 1 ounce of concentrated cannabis and up to 72 ounces of edibles
  • Cultivate up to 6 mature plants and 6 seedlings per patient in their care
  • Transport and administer medical marijuana directly to their patient

Possession limits are cumulative per patient served. A caregiver serving multiple patients may possess the sum of limits across those patients’ licenses.

Documents You’ll Need to Apply

Before beginning the caregiver application, make sure you have the following ready:

  • Valid government-issued photo ID – Oklahoma driver’s license, state ID, U.S. passport, or approved tribal ID (front and back)
  • Proof of Oklahoma residency – current utility bill (not a cell phone or internet bill), property deed, or lease agreement
  • Passport-style digital photo – plain, pale, or white background, full face visible, no glasses or hats
  • Completed Caregiver Designation Form – signed by the patient (or by the minor’s parent/guardian for minor patients); this establishes the official patient-caregiver relationship.
  • Patient’s OMMA license number – you’ll need the patient’s full medical marijuana license number to link your application to their.

The patient must already be licensed with OMMA before you can submit a caregiver application. Their physician’s recommendation must also include the section certifying the need for a caregiver.

Step-by-Step: How to Apply for Your Oklahoma Caregiver License

Step 1: Confirm the Patient Is Already Licensed and Has Caregiver Certification

However, before you do any of that, ensure that your patient already has an OMMA license and that the physician recommendation form contains a caregiver certification part. If not, your application would be denied. If the patient still requires his/her card or his/her physician recommendation form with a caregiver designation, then you can conduct the telemedicine appointment at oklahomacannabiscard.com.

Physician Evaluation Cost: $129 per 2 Years – It is included in the patient’s recommendation, with the caregiver certification section included, if required, for two years. 

Step 2: Obtain the Signed Caregiver Designation Form

You and the patient must both complete and sign the appropriate Caregiver Designation Form:

  • Adult patients – use the Adult Patient Caregiver Designation Form, signed by the patient
  • Minor patients – use the Minor Patient Caregiver Designation Form, signed by the minor’s parent or legal guardian

Download these forms from the OMMA website at oklahoma.gov/omma. Have your digital copy ready to upload before starting your application.

Step 3: Create Your OMMA MedPortal Account

Click here to access the OMMA MedPortal at oklahoma.gov/omma, and establish a new account by using your email address. In case you already have an account from previous applications, you should log in with your existing credentials. Please use Chrome, Edge, or Firefox browsers; IE is not supported.

Step 4: Complete the Caregiver Application

Once logged in, navigate to the caregiver license application. You’ll be prompted to:

  • Enter your personal information (name, date of birth, address)
  • Enter the patient’s OMMA license number to link your application
  • Upload your photo ID (front and back), proof of residency, and passport-style photo
  • Upload the signed Caregiver Designation Form

Verify that all information is accurate when compared to your supporting documentation, and that your photograph complies with OMMA’s standards of having a blank background, visible face, and no glasses or headgear.

Step 5: Pay the Caregiver License Fee

A caregiver license from the OMMA costs nothing, as it is free to apply. This is one of the most accommodating features of Oklahoma’s medical marijuana program for patients.

Step 6: Wait for OMMA Approval

It takes 14 business days to process an application for a caregiver license from OMMA. Your license will arrive by mail, and you will receive notice of approval. Once you have been approved, you may use your caregiver license at any authorized dispensary in Oklahoma to buy marijuana for your patient.

Renewing Your Caregiver License

Caregiver licenses last two years, corresponding to when the patient’s license expires. Once your patient’s license expires and is renewed, your caregiver license must also be renewed through much of the same procedure, including the signing of another Caregiver Designation form. Should the patient’s license expire or become revoked for whatever reason, the caregiver’s license becomes voided alongside it.
